PERSONAL DATA COLLECTED BY OSPITAL NG MAKATI
The doctors, nurses and team of healthcare professionals caring for you keep records about your health
and any treatment and care you receive from us. These records help to ensure that you receive the best
possible care.
They may be written down in paper records or held on computer. These records may include:
• Basic details about you such as name, address, date of birth, next of kin, etc.
• Contact we have had with you such as appointments or clinic visits
• Notes and reports about your health, treatment and care
• Results of x-rays, scans and laboratory tests
• Relevant information from people who care for you and know you well such as health professionals and
relatives
It is essential that your details which we hold are accurate and up to date. Always check that your personal
details are correct when you visit us and please inform us of any changes as soon as possible.
REASONS FOR COLLECTING THAT PERSONAL DATA
We collect your Personal Information primarily to provide medical services. We also provide information to
our clients, partners and service providers for the same purpose.
We may also use your Personal Information secondarily, in circumstances where you would reasonably
expect such use or disclosure of your Personal Information.
You may withdraw your consent (subject to the applicable Privacy Laws) to the processing of your Personal
Information at any time by contacting us in writing. We provide below the general categories of the purposes
for which we collect and process your Personal Information.
RECIPIENTS, TO WHOM YOUR DATA MAY BE DISCLOSED
The following will have access to your Personal Information for the purposes stated above:
a. Healthcare professionals, including, without limitation, the members of the Hospital's medical staff,
nurses, caregivers and other healthcare providers, either pursuant to an employment contract or any
other arrangement. They will have access to your Personal Information either because they are
authorized to enter information to your medical records, review or update the same.
b. All departments, section and units in the Hospital who will need your personal information in the
performance of their functions. For example, certain treatments or procedures require that your Personal
Information be shared across different departments of the Hospital.
c. Any member of a volunteer group, religious or charitable/ non-profit organization who are allowed to
provide assistance in the Hospital. This includes priests, pastors or heads of other religious
organizations who provide religious rites to patients or the deceased.
d. All of our employees, staff or personnel who may need access to your information in the performance of
their duties. For example, our employees will access your Personal Information in order to prepare your
billing statement, or your medical information will be needed for your dietary needs during your
confinement.
e. All entities operating within the premises of the Hospital. The Hospital may share your personal
information with these entities for the purposes stated above.
We will not disclose your information to any other third parties without your permission unless there are
exceptional circumstances, such as if the health and safety of others is at risk or if the law requires us to
pass on information.
You have the right to restrict how and with whom we share the personal information in your records that
identifies you. This must be noted explicitly within your records in order that all healthcare professionals and
staff treating and involved with you are aware of your decision. By choosing this option, you should be
mindful that it may make the provision of treatment or care more difficult or unavailable. You can also
change your mind at any time about a disclosure decision.
RETENTION OF PERSONAL DATA
We retain personal data for no longer than required and in line with OsMak's retention schedule. This is
based on statutory requirements and legal obligations, as well as our business requirements.
SECURITY OF PERSONAL DATA
We take our duty to protect your personal information and confidentiality very seriously and we are
committed to taking all reasonable measures to ensure the confidentiality and security of personal data for
which we are responsible, whether computerized or on paper.
PERSONAL DATA AND YOUR RIGHTS
Data protection legislation gives you the right to:
• Correct any data we hold about you that is not correct (Rectification)
• Request that we delete your personal data (Erasure)
• Block or suppress the further processing of your personal data in certain circumstances (Restriction)
